چکیده
PURPOSE This study aimed to compare the overall and disease-free survival of patients who underwent video-assisted thoracoscopic esophagectomy (VATE) or open esophagectomy for esophageal carcinoma. METHODS Patients who underwent radical esophagectomy via VATE (VATE group, N=89) for esophageal carcinoma between January 2008 and December 2012 were retrospectively enrolled in this study. Patients subjected to open radical esophagectomy (open group) were retrospectively included at a ratio of 1:1, matching the VATE group in sex, age, clinical TNM stage, location of the primary tumor and ASA (American Society of Anesthesiologists) score. RESULTS All the video-assisted thoracoscopic procedures were successfully completed, without conversion to open procedure. The age, gender, clinical TNM stage, location of the primary tumor and ASA score were similar in the two groups. VATE group was associated with significantly less blood loss and shorter hospital stay. The operative morbidity and mortality were similar between the two groups. The quality of the specimens in terms of resection margin and the number of lymph nodes examined were not inferior in the VATE group. With the median follow-up of 52 months, the 5-year overall survival and disease-free survival were similar between the two groups. The operative approach was not an independent prognostic factor in the overall and disease-free survival in univariate and multivariate analysis. CONCLUSIONS VATE for esophageal carcinoma is associated with more favorable short-term outcomes and comparable long-term prognosis when compared with open esophagectomy.

BACKGROUND AND OBJECTIVES The safety and effectiveness of minimally invasive esophagectomy (MIE) in comparison with the open esophagectomy (OE) remain uncertain in esophageal cancer treatment. The purpose of this meta-analysis is to compare the outcomes of the two surgical modalities. The aim of this study is to evaluate surgical results and long-term survival of combined thoracoscopiclaparoscopic esophagectomy (TLE) performed for esophageal squamous cell carcinoma. Data of 59 patients with esophageal squamous cell carcinoma, undergoing TLE from January 2007 to January 2015, were compared to a control group of 59 patients who underwent open esophagectomy (OE) during the same...
متن کاملMinimally invasive management of chylous fistula after esophagectomy.
Chyle leak is an unwelcome complication of esophagectomy that is associated with a high mortality. The diagnosis of this condition may be difficult or delayed and requires a high index of suspicion. Management varies from conservative treatment with drainage, intravenous nutrition, treatment and prevention of septic complications, to re-operation, either by thoracotomy or laparotomy to control ...

متن کاملThoracoscopic Surgery in a Patient with Multiple Esophageal Carcinomas after Surgery for Esophageal Achalasia
We present a case in which we used a thoracoscopic approach for resection of multiple esophageal carcinomas diagnosed 33 years after surgery for esophageal achalasia. A 68-year-old Japanese man had been diagnosed with esophageal achalasia and underwent surgical treatment 33 years earlier. He was examined at our hospital for annual routine checkup in which upper gastrointestinal endoscopy showed...
